North York

The Patient Service Centre (PSC) at 57 Gervais Drive in North York is dedicated to offering specimen collection services to Naturopathic patients. Our phlebotomists are trained specifically on all test kits available to order through ICL. Appointments can be booked by calling 416-422-3000 ext 300, ensuring your patient will experience little to no wait time.

Your patients can pick up test kits for self-collection from the North York PSC. Please call or email ICL Client Care to ensure we have the kit in stock. This will enable you to store less kits in your office and can expedite the testing process as you and your patient will not need to wait for test kits to be ordered and sent to your office.

The ICL North York PSC Hours of operation are Monday to Friday from 9:00 am – 5:00 pm.

PLEASE COLLECT SAMPLES ACCORDING TO THE INSTRUCTIONS IN THE COLLECTION KIT AND THEN SEND SAMPLES TO ICL USING THE INSTRUCTIONS BELOW.

Important: Please label specimen containers with patient’s full name & date of birth
before shipping to ICL.Name on specimen containers and requisition must match returning samples for testing.

Option 1:
–  Call Purolator at 1-888-744-7123 and select 1 for English or 2 for French
– Say “Schedule a pick-up within Canada” and press the ‘0’ key when asked to
provide a locator code, extension, or Purolator account until you are transferred
to a Customer Service Representative

Option 2:
– Click the Chat option at the top of the page
– Type “Schedule a pick-up”
– Follow the prompts in the Chat window

PLEASE NOTE: Purolator will not pick up from your home late on a Friday, Saturday or Sunday. At home pick up is best done on Mondays to Thursdays (excluding
statutory holidays). Schedule your pick-up in advance to prevent delays.

To order kits to be delivered to your practice location, please email results@iclabs.ca

Please include the following information:

  • Practitioner Name:
  • Address for Kit Delivery* (including unit/apt #)
  • Test Name(s) and quantity

*Please ensure the address information is correct to avoid any additional charges for missed deliveries due to an incomplete address.

To order kits to be delivered directly to your patients please follow the steps below:

IMPORTANT: We are unable to accept supply requests via email for direct-to-patient shipments as it may present a breach of privacy.

 We ask your cooperation in this matter to ensure that orders can be processed expeditiously while maintaining required documentation and privacy protections.

How to Proceed Securely:

  1. Contact ICL by telephone or fax to request this service. Please identify the patient, the test, and delivery address for the kit. Call: 416-422-3000 x 444 or Fax: 416-385-1957
  2. ICL will ship a kit to the patient at the address you provide. Please advise your patient they need to be home to receive the package.
  3. At the same time, ICL will send you the required test requisition (by email or fax, whichever preferred). Please complete the requisition as usual and provide the completed requisition to your patient to include with their sample when returned to ICL by Purolator or Canada Post.
  4. Upon receipt of the collected specimen and the completed signed requisition, ICL will process the order to the testing lab.

Note that you are requested to adhere to strict privacy protection guidelines by not transmitting personal health information (such as completed requisitions, patient identification and requested tests) via the public internet/email system; you should only provide such information by postal mail, to our secure fax unit, via telephone or any secure email service you have subscribed.
