Colorectal cancer screening with fecal immunochemical test (FIT) at In-Common Laboratories (ICL)
The FIT is an at-home, safe and painless test used to screen people at average risk of developing colorectal cancer. Specifically, the FIT checks for the presence of occult blood in the stool, which can be an early sign of colorectal cancer or pre-cancerous lesions.
ICL is the central laboratory managing the distribution and testing of FIT kits for the ColonCancerCheck program in Ontario. Primary care providers should fax the new ICL FIT requisition to 1-833-520-1544. Instructions for how to complete the FIT requisition are available here.
Find more information in the drop-downs below:
ColonCancerCheck’s new guidelines are in effect starting July 1, 2026. These guidelines include the lowering of the colorectal cancer screening start age to 45.

The new FIT requisition for use with ICL is available here. It is also available on Ontario Health’s (Cancer Care Ontario) website along with requisition instructions and other resources: ontariohealth.ca/coloncancercheckhub
The FIT requisition is also available in all OntarioMD-certified electronic medical records (EMRs).
You can now order a FIT kit using the In-Common Laboratories (ICL)-branded FIT requisition for eligible, average risk people. Completed FIT requisitions should be submitted directly to ICL by fax to 1-833-520-1544.
You should ensure that all required fields in sections 1, 2 and 4 are completed on the FIT requisition to prevent any rejections or delays. Guidance on how to complete the FIT requisition form are available here.
Yes, in June, In-Common Laboratories (ICL) mailed samples of the new FIT device to primary care providers so they could show patients how to use it. However, if ICL did not have your complete mailing address, you may not have received one.
If you did not receive a demonstration FIT device and would like one, please complete the form on this page: iclabs.ca/FIT/demodevices
An In-Common Laboratories (ICL) account is required to get FIT results. In most cases, ICL handles the set-up for you.
If you have submitted a FIT requisition to LifeLabs within the past two years, ICL will automatically create your account. If your contact details need to be confirmed, ICL will be in touch.
If you have not submitted a FIT requisition to LifeLabs within the past two years, ICL will create an account for you the first time you submit a FIT requisition to ICL.
Please note that account set-up is required for each of your working addresses or locations and fax numbers. If you practice at multiple sites, use multiple fax numbers or work as a locum, each location will need a separate account, and each fax number must be verified separately.
Fax verification
If you have not submitted a FIT requisition to LifeLabs in the past two years or ICL had to confirm your fax number, you will need to complete a one-time short fax verification step to get FIT results:
- Watch for the verification form ICL will fax to you, and
- Sign the form and fax it back to ICL.
Note: ICL will still mail FIT kits even if fax verification is pending.
Fecal immunochemical test (FIT) results are available in the Ontario Laboratories Information System (OLIS), but you must also have a direct method to receive FIT results. Fax is the default. Ontario Health (Cancer Care Ontario) is working with OntarioMD-certified electronic medical records (EMRs) to make FIT results available directly in EMRs.
In-Common Laboratories (ICL) will send FIT results via fax by default. See below to learn more about FIT result reporting options.
OntarioMD-certified EMRs
- Ontario Health (Cancer Care Ontario) is working with OntarioMD-certified EMR vendors to make FIT results available directly in your EMR. Because not all OntarioMD-certified EMR vendors will offer this functionality at launch, FIT results will be sent by fax in the interim. If EMR is your preferred reporting method, please indicate which OntarioMD-certified EMR you are using by completing the form at iclabs.ca/FIT/reports. If you complete this form, In-Common Laboratories (ICL) will switch your reporting method from fax to EMR once the feature is available in your EMR.

- Accuro® EMR – QHR Technologies
